This page covers the website only. How we handle your health records at the clinic is described in our Notice of Privacy Practices, which you can ask for at the front desk.

What the visit request form collects

• Your name and phone number, and your email address if you give it.
• Whether you're new or returning, the reason for your visit, and the days and times that suit you.
• Any note you add. Please don't include medical details in the form; we'll ask about those on the phone.

Online booking

When you book with the booking assistant, we save the appointment time, visit type, names, phone number, email (if given) and any note on our web server, and email a copy to the front desk. Past bookings are removed regularly. The assistant is automated; it doesn’t record anything you don’t send.

How we use it

Only to call or email you back about an appointment. We don't sell it, share it for marketing, or add you to a mailing list.

Where it goes

The form sends an email to our front desk. Requests are deleted from the inbox once they've been dealt with.

Cookies and tracking

This site doesn't use advertising or analytics cookies. The form uses one short-lived cookie to stop repeated submissions. Fonts and all other files are served from this website, not from third parties.
